Dual Enrollment (Government at North Lake College over Summer)
DEADLINE: April 27, 2020
Use this checklist (https://www.dropbox.com/s/yur8dw0eiqp2dt1/Dual%20Credit%20Check%20List.docx?dl=0) to ensure that you have completed all the necessary steps. On the checklist, the first step las a link to complete the online application on DCCCD's website. Once you complete the online application and get accepted, you will receive a student ID that will be necessary to fill out the supporting documents shown linked below.
(1) Complete the 3 attached documents. You can download these and edit them/fill them out directly in Word:
Dual Credit Enrollment Form (https://www.dropbox.com/s/fbp38z5rafkmldw/Fillable%20Dual%20Credit%20High%20School%20Enrollment%20Form%202020-21.docx?dl=0)
FERPA Release Form (https://www.dropbox.com/s/ejeycxic8jwfn33/ferpa-release%20form.docx?dl=0) and copy of proxy’s photo ID
Consent for Emergency Medical Treatment (https://www.dropbox.com/s/xu6iadbz8m4efvb/Fillable%20consent-emergency-medical-treatment%20%281%29.docx?dl=0)
(2) Compile completed documents + copy of proxy’s photo ID into ONE PDF document and email to your academic counselor by April 27, 2020. One possible way to do this is to export your Word documents to PDFs (go under File, Export, Create PDF);
If you have Word documents, you can export them to PDFs by going under File, Export, and Create PDF OR you can save as PDF by going under File, Save As, and selecting PDF from the dropdown. For more assistance, contact me or visit https://support.office.com/en-us/article/save-or-convert-to-pdf-or-xps-in-project-desktop-d85416c5-7d77-4fd6-a216-6f4bf7c7c110
Free apps such as Genius Scan or even the Notes app in iOS (https://support.apple.com/en-us/HT210336) can be used to scan documents into a PDF from your smartphone if you don’t have access to a scanner.
You can merge your documents into one PDF by using an online PDF merger such as https://www.ilovepdf.com/merge_pdf
As a reminder, academic counselors are Ms. Sanders (csanders@uplifteducation.org) for last names A – K and Ms. Stafford (estafford@uplifteducation.org) for last names L – Z.
All parent and scholar signatures are required for a complete application. The school will submit the required transcript electronically upon receipt of the completed packet. Please reach out to your Academic Counselor with any additional questions.
I have 4 separate pdf files. I need to print content in those files on a single paper like in below image.
